The membership form is designed in 2 parts, and can (if needed) be printed out and folded together: the outer pages contain the subscription bands information (which is liable to change); the inner pages are the form itself (which is not liable to change).

 

The form has been set up so you can type within it, but it will still then need to be printed off in order for it to be signed - NB it will need 2 signatures: one on page 2 and then another on either page 3 or 4 (Direct Debit or payroll). Once signed, it can be scanned and then emailed back to acts.unitele372.membership@gmail.com
